The Regional Partnership Catalyst Program (RP) was developed to foster widespread collaboration between hospitals and community partners and build infrastructure needed to sustainably support the population health goals of the Total Cost of Care (TCOC) Model. The program was designed to address the diabetes and opioids population health priority areas under the Statewide Integrated Health Improvement Strategy (SIHIS). The funding is narrowly focused to support the implementation or expansion of diabetes prevention and management programs, as well as behavioral health crisis programs. The Regional Partnership Catalyst Program began January 1, 2021 and will conclude December 31, 2025.
