The Vaccine Tracking Service is a web-based application that enables physicians and payers to track their patients’ vaccination status and prioritize and outreach to unvaccinated patients. The reports are populated using daily vaccination data from ImmuNet, Maryland’s Immunization Information System (IIS), ENS Panels or MDPCP Practice lists for each provider’s attributed population, and patient Chronic Condition flags based on administrative claims data.
Summary level reports help practices understand the proportion of patients that have been vaccinated by demographics to ensure their population is vaccinated over time and compare their performance to relevant peer groups. Users can use the Patient Navigator to segment their population by age and chronic conditions to focus outreach efforts, and customizable fields allow users to document outreach notes.
CRISP has expanded the utility of the Vaccine Tracking Service to now include the Flu vaccine. Users can now toggle between COVID-19 and the Flu Vaccine to track and prioritize patient outreach.
