Readmission Reports

Overview

The CRISP Reporting Services hosts a few types of reports built for users to understand hospital performance under the HSCRC’s Readmissions Reduction Incentive Program (RRIP). Below is a brief description of each type:

1. Readmission Summary & Patient Level Detail Excel Files – are excel based report run in SAS using the inpatient Case-mix data (run under APR-DRG grouper v38).
2. Readmission Tableau Reports – are inactive reports which allow users to filter and view graphically the performance and base period readmission data. These reports contain the exact same data as the excel report, however, they provide users additional insights and comparison options. The Readmissions report includes 11 tableau-based reports, which are populated using the inpatient Case-mix data (run under APR-DRG grouper v38) and out-of-state readmission rates from Medicare data obtained from CMMI.
3. Patient Adversity Index (PAI) and Disparity Gap Report – can be used to track within-hospital disparities in readmission rates. The HSCRC staff developed the Patient Adversity Index (PAI), a composite social risk index incorporating information on patient race, Medicaid status, and the Area Deprivation Index (ADI) for the area surrounding the patient’s address (as recorded in claims).
4. Excess Days in Acute Care (EDAC) –The EDAC measure is an alternative to the traditional readmission measure, which assesses the time spent in the emergency room, observation status, and inpatient status in the 30-days after discharge.  Medicare reports these measures on a condition specific basis, however we have developed through our contractors an all-payer, all-cause measure.  As part of the RRIP Redesign under the TCOC model, stakeholders agreed this measure should be developed since it is patient-centered, captures ED and observation revisits, and assesses severity of readmission.
